Can anyone please provide me format for affidavit of father name change.
Here is the complete matter.
My Sister is studying in 10th standard. my mistake during form fill up father name had been change from "Hardayalsinh" to "Hardayalbhai". I had word with school staff they are asking me to provide affidavit for the same.
So here can anyone please provide me format for the same?
