my query is, can a person who has changed his name and religion use his/her past name/religion to open a bank account etc. ? if not, what action (and to what extent) can be taken against that person ?

How has the name and relegion been changed?
If the legal name has not been changed by following due procedure, there is no problem in opening bank account on the same.
