Requirements for witness submission

In a State case, the OC of a police station has written in the witness portion of the Charge Sheet, "as told to me by the witness". Then the witness pages have been written in somebody's hand writing (probably the complainant's) and which is not the witnesses' and signed by the O.C. himself. There is no signature of the witnesses anywhere. None of the witnesses has ever shown up in court either.

My question is that is this legal? Can O.C. say that I am writing on behalf of witnesses and based on the oral deposition given to me by them? Witnesses signature is also not required?

P.S. Sorry this should be in CRIMINAL LAW section of this forum. Gave in civil law by mistake. Nevertheless all experts are pretty knowledgeable and so it should not matter.
