Resignation from psu bank

Sir/Madam,

    I am working in psu bank as asst manager (RDO) SINCE 2011.I have completed the bond period & now i am confirmed employee of the bank.

now i am on leave for more than a month. I dont want to continue in that organisation. I want to put my resignation. in my appointment letter it is mentioned that in case of resignation u need to give three months of notice but it is nowhere mentioned about the notice period salary in lieu.

Now i am not in a position to serve the notice period.  So is there any chance to tender my resignation under medical grounds. As an alternative i am thinking of starting a business of my own.

I am having following queries in this regard:

1)as i have already submitted my medical certificate for above said sick leave, what is the procedure for my resignation under medical grounds for not serving the notice period?should i send my resignation letter via registered ad/email/in person?

2)if i don't serve the notice period will they issue me NOC, EXPERIENCE CERTIFICATE & RELIEVING LETTER?

3)Is it advisable to go on medical leave during notice period?will they issue me  NOC, EXPERIENCE CERTIFICATE & RELIEVING LETTER after 3 months?

4) will i have to pay 3 months salary in lieu if i don't serve notice period?

5) what will be the probable losses to me if i don't serve the notice period?
