Please approach a lawyer. Publish name change in gazette or in news papers as per the form. Details of the format are available with Passport Office or your lawyer will guide you. Name correction shopuld mention your name as in Aadshar & PAN Card.

After the correction is published you have to approach the Passport Office to change the name in Passport as newly published.

First, Please file application , seeking information under RTI Act,  the proper procedure for correction of name as per PAN card or AADHAR and other degree certificates if any as per laid down norms/directives/notifications and seek a copy of such procedure.

There is no specific format and you have to file application to CPIO, PPO....with RTI Fee of Rs.10/- through Indian Postal Order for Rs.10/- Fvg. Accounts Officer, PPO, payable at PPO's station.

After receiving the procedure, implement those directions in circular.  May be some minor official requirements, as AADHAR is now a bonafide number for several purposes.

Name i the SSC certificate cannot be changed now, but the same can be changed in the Aadhar card and PAN card.

For change of name in  the passport,  an affidavit as per  Annexure E should have been attached along with the passport application  at the time of  applying for it, now you can enquire from the passport office about the procedure to be complied with in this regard.
