Property registration

Recently I got the Property Registration done in the UP, I have just received the document and found the following gaps, please suggest if these impact the correctness of the legal document 

1. Advocate has family name in the  stamp is wrong instead of Kumar it is Kuamr, does it impact document.

2. Boundary of plot is mentioned as per site plan and site plan document is missing, but Layout of flat is attached.

3. Witness Id proof is missing in the registration document.

Please suggest should I go for rectification  deed in this case or these can be igonred

Yes. It is better to go for a rectification deed.

Wherever any  rectification becomes pertinent, it would be advisable that a rectification deed may be executed.
