Proving contents and statements therein

Law of evidence speaks about proving the contents of the document by producing primary evidence.  Does this contents mean  only alpha-numeric characters or the meaning thereof also?  Secondly, does this mean that meaning stated therein  is true in fact hence proved (eg A murdered B! Does this mean A realy murdered B)? What would be the position when the document is public and private certificate (emphasis)?
