Dear All,
My earlier PAN card was having typo-error and based on it, bank account was opened. Now, i have rectified it on PAN card and same changes wants on Bank account details.
As per Bank, I need to make an affidavit on it. Can you please suggest me, what are all i need to carry to make an affidavit? What would be the charges to make it (as per kolkata region)?

10/- non judicial stamp paper and the details what was the first name printed in the pan card and after corrected what was the name changed and documents to show the correct spelling of name to show and also verify with bank athorities if any format is there with them and submit
