what is the format of application to be filed before the magistrate u/s 144 of criminal procedure code,1973,regarding urgent cases of nuisance and apprehended danger

Regular format using in court.... file the petition with affidavit , vakalat to JM or CJM or Executive MAgistrate like RDO or Dist Collector
