Civil suit

i have filled a civil suit against bank for getting back the service benefits and mentioned 30 enclosure documents for proving my claim and given out the affidavit after that i given out form no 3 and enclosed 30 document but unfortunately the document no 26 is being different in the list of form no 3 and affidavit the document mentioned in form no 3 is enclosed but differ the document mention in affidavit how can now this deficiency can be remove
