I need to apply for a passport for my Father. there are couple of situation in his case:
1. He is seperated from last 25 yrs and he dont have either marriage or Divoce certificate.
2. His School leaving certificate is with name1, his voters id card is with name2 and his name in my school certificate is with name3.
3. I want to have his passport with name3.
Can anyone please suggest the overall process to achieve this?

you can apply for passport with the name of school certificate,along with your father bank passbook,ration card an school certificate as proof.
