Disability certificate

Dear Professionals


My mother aged 65 is met with accident approximately 30 years ago.  She was hurt in her spinal card and lost her both legs functioning and she is using wheel chair since than. Practically she is disable.  

However, we did not applied any disability certificate from Govertment authorities.  Please advise me where and whom to be approached to get such formal disability certificate. 

Presently she is living in Chennai, Tamil Nadu. Kindly help me.

The medical board of doctors authorized by the Chief Medical Officer conducts assessments and ascertains the percentage of disability for the disability certificate. Any person with a disability (may be physical, visual, speech and hearing or mental) of more than 40%, and who has a disability certificate is entitled to avail all the government services for the disabled.
