Getting name of the son along wit father in a new patta land

Sir My father has got 2 Full plots from different two Khatas by virtue of two relinquish Deeds. Now what Process do We need to get Patta in my father's name for the same piece of land. 1.Do I need to apply separately for both Plots. 2.Is it Possible to get my name recorded along with my fathers' name in the new pattas. (what is the Process). 3. My father has 03 Sons (no daughter). Any Special procedure cause I want only my self along with my father to be co sharer of the new Pattas. I am from Odisha.
