Private telecom companies activate some services by default e.g. ISD etc without obtaining explicit consent of customer, at the time of booking the connection. Whereas BSNL shall obtain extra security, consent on separate form, and PN card number for activating ISD.

You are within your rights to demand the usage report in writing from company.

Did you ask for issuing complaint number to you while reporting to CCE, senior staff? Did the company CCE or staff decline to issue complaint number to you?
It shall be appropriate to lodge a complaint in writing by letter (notice which you can also send) thru redg. post, addressed to the MD of the company narrating and covering all points and representations. You may mention that reply should be supplied by letter thru redg. post only under original seal and signature of the competent employee of the company and no telephone call by any staff should be made to you and no email should be sent to you, and that a self addressed postage prepaid envelope for sending reply by redg. post is enclosed with your letter.
It shall be appropriate to contact a competent and experienced lawyer handling consumer cases and submit structured representation to RM of the company sitting in RO of the company in your location and MD, CS etc. You may meet RM in person and obtain acknowledgment on copy of your letter.
