As per the situation you need to file a petition for an 'Order of Protection' in a court of law that has competent jurisdiction over the matter.
Moreover, you have to sign an affidavit about what your father's abusive actions that he has done to you.
However, remember that an affidavit is a form in which the an individual swears all the information to be true and had to sign in front of a notary or a Judge. Thus, state only true facts and issues in the affidavit.
But, I would suggest that being an adult you should first try to resolve the matter with you dad within the house itself before approaching to the court because there are an end number of possibilities that the situation between you and your father could improve and you might not even need any protection.
Although, if in case the situation does not seems to improve then you may take the matter to the court.
