Approach your bank branch where from you obtained the DD. They themselves would give you the Indemnity Bond Format. While the form given by Mr. Khan is OK, yet the Bank concerned would be insisting on the Bond in the format which they have. Better check up.

Agree with Ramachandran. I am a banker myself, the format is available with each bank which is different in every sense. Please visit the bank for the format.

i need to know how can i apply for a birth certificate , the hospital which i born was closed long before , am in need of birth certificate because one of my friend told me that for applying passport birth certificate is very much needed. pls help me

In case of non availibility of birth certificate, get an affidavit of your mother prepared duly executed before, notary or executive magistrate.
