Legal documents to be submitted for getting maternity leave

What are the legal documents that should be received from a hospital before getting the baby. When applying for maternity leave/adoption leave, what are the legal documents that need to be submitted to an organization.

The admission of an "Inpatient Certificate" for the purpose of delivery, is enough for you, to get  maternity leave, from your Organisation. .
