Mact case sine die - for revival

Dear Fraternity :

In one of a case ( MACT) in the District Court Delhi, the case has been declared to be Sine Die. The same has to be revived.

1. An applicaation format is required for this. Could anybody post the same or its URL, while being anwered. Please also let me know

2.what kind of documents required and

3.court procedure.

If the case is dismissed in default and not on merits then you can file a new case as there is period of limitation. Restoration application can also be filed.

Thank you very much.

I have spoken to the Reader in the Hon'ble court, He has suggestted that " A revival pettition / application" has to be filed. I need to have the format on which the restoration can be made.

And what is the limitation of period upto which the same has to be filed. This particular case was put on " Sine Die" on 07th / November / 2013.
