Name change

My daughter got a divorce from her husband in March 2017 when her son was 11 months old. We have not asked for any alimony and the child's father has no visitation rights. My daughter had not changed her maiden name after marriage. All legal documents like PAN card, voter card, driving license, aadhar, passport, bank accounts, etc are in her maiden name. We now want to change the entire name of the child. First name, middle name and surname. Please advise us the procedure.
