Need format of application to submit additional documents (by respondent) in an slp before admission

Dear learned members

I am a respondent in an SLP filed in Hon'ble Supreme Court. The petitioner has not filed some documents that were part of proceedings in Hon'ble High court, and these are important documents.

I want to submit these documents before the admdission hearing scheduled in January. 

I am looking for help for the application format that I will need to use to submit these documents.I will be submitting them through efiling portal.

I will be very thankful if you can send me a draft/sample of application that I can use.

Alternatively, I am attaching here a format that I have drafted for submission. If you can let me know if this is ok, and changes to me made, I will really appreciate.

I believe I will have to use application with affidavit and the documents as annexures. If there is anything else to be submitted, pls let me know.

You can contact your supreme court lawyer who filed your vakalatnama before supreme court in this case,  for knowing the format as well as the procedures to file documents from your side.

Actually you will be required to file a counter objection at the time of admission, you can enclose the copy of he documents you rely upon as your type set of documents or you can enquire the procedures from your advocate.

Whether notice has been issued to respondent of SLP by Supreme Court ? If so, SLP is presumed to have been admitted to be considered as an appeal and provision of appeal shall apply.

In such circumstances you (respondent) are required to file your reply to the appeal and move an interlocutory application for filing additional document(s), which did not form part of proceeding before High Court. 

What is the opinion and advise of your lawyer, follow it.
