Dear sir,
my wife filed 498a, dvc and maintance case. I got maintanance petition filed by wife. It is crpc 125 section. I need to File counter for this. My lawyer asked me to prepare myself. Please help me to provide sample format of the maintanance case counter.

It is duty of the advocate to prepare your reply after consulting with you.
If he/she said you prepare yourself, it mean you need to change your lawyer.

It is a surprising response, the lawyer mat have asked your inputs in written form on all points, but asking you to prepare a reply. Surprising.

first give all the inputs from your end point wise and if he insist you to prepare the reply by your self then change him as said above
