Note the date when for the first time you has reported it in writing. Since then was he sleeping. While he does not forget to collect salary, how can he forget to do what shall fetch him his salary.
Now both have to reply who was designated employee/reporting authoprity, to pass his mediclaim. At the time he passed his own mediclaim, was he instructed to do so ,if yes by whom.
What penultimate action,on whcih date, has been taken by designated employee/reporting authoprity,Chairman,and Minister,and what report has been filed. What has been ordered and to whom in writing, by Chairman and report.
It is obvious that the chairman is neither preturbed,nor disturbed,nor concerned, and is not acting, hence should be taken to task.
If the minister has not taken any firm action, then escalate to his reporting authoprity,PM,President.
