Property transfer after death

Dear Sir,

My father was died before 3 years. He had not done joint sale deed with my mother. The file of property is mortgage with Bank. Now we are planning to pay loan to bank and take file from the bank. 

What is the procedure to add my mother's name in Sale Deed because my father had executed sale deed but only his name, he had not joined any of our name.

Please guid me?
