You can use the affidavit format of passport office for minor name changes. Then you can get it published in form of a public notice in local newspapers. You have not specified which type of document,,,,,even then if the wrong entry is in any public record/educational record...u have to do these two things at least.

As the documents pertain to you, the deponent of affidavit should be you only, not your father.

If the affidavit is found to be error by spelling mistake, the deponent, who eve it was, can either go for rectification or fresh affidavit.  You can contact a Notary public in the local who will be able to guide you further on this issue.
