Substitution of legal heirs/representative

Respected Experts,

A person was convicted by a lower court while he was serving in a Central Govt. Deptt. Thereafter he was granted bail from the Hon'ble High Court.  However, due to his conviction in a criminal case, the disciplinary authority dismissed him from service under Rule-19(i) of the CCS(CCA)Rules,1965. He sent departmental appeals, but rejected by the Appellate Authority.  Threafter, he filed an OA before the CAT, and during the pendency of OA, the person has now died.  The brief of the case is that the disciplinary authority (before issuing the order of dismissal) had not provided an opportunity of making representation on the penalty proposed to be imposed before any order is made.  Now the widow/children of the deceased peson (Applicnat) would like to file an Application to bring legal representative of the deceased person on record under Rule-18 of Central Administrative Tribunal (Procedure) Rules,1987.

I request the Hon'ble Expert Members kindly to provide a draft format of application for the above purpose, and oblige.
