Reliance Mutual Fund had created an fake ecs mandate form that is contains my name my pan card details and the signature bearing the document was forged and the nominee for the mutual fund account and the address on the form was also false.
 
The form was having the account no ending with 124689 and my account was in Punjab National Bank Account no ending with 123689 so there is error in account no then also the Punjab National Bank Allowed to RMF (Reliance Mutual Fund) To charge my account.
 
Now here both the RMF & PNB are in fault and they had incurred loss of 2100/- INR to me in 2007 and now I need to have the same to admit the case to both of them firstly the RMF had used my pan card details and my signature had forged with intention to charge my bank account and my bank also turned there eyes close and in-spite there was error in the account no they had allowed the RMF to have the amount from my personal savings I am having all the related documents that will prove that the RMF and the PNB had incurred me with the loss in Financial state.
 
So i am in touch with the RMF and PNB for settlement but they are not responding now its out of control and I need to file the legal case on both the parties......

You may issue a legal notice demanding explanation from the concerned office about the irregularities pointed out.  Subsequently you may initiate necessary legal actions through court.
