i am an individual, if i donate money to a hospital or any other charity work how can i declare that money/donation in my ITR ?

If the organization to which you donate money is a charitable organization, then they will issue Certificate under Sec. 80G of the Income Tax Act.
On the basis of the said 80G certificate, you will be able to claim 50% of the donation as expenses. (not 100%).
