Accident cases check list - esic

Hi,

 

Can you please provide me the updated check list for Accident cases in ESIC, in case of an accident of employee on during or off work.

Dear  Madam

 

In case of an accident of employee on during working in the company

1. Pl inform the ESIC Local Office and District office with employees details

2.  subsequent letter  Hospital name, address

3. Accident details

4. I F intimation letter form no 19

5. Medical Bills with claim form

 

or off work.
