If CMD, GM(P), all by name ,are not initiating action on Ex. competent authority(even if she has left (NIACL), and bribe taker caught red handed, then you may submit complaint to Director Insurance/Under Secretary by name, and if he/she also has not acted beyond mere pushing papers, you may address the complaint to PM by name, and if still nothing happens then you may  approach institutions of print and electronic media, court of law, and expose the jungle raj.
