The transactions by Bank transfer are for Dec15 only.

The Form16 as usual is from; 01st April 2015-31st March 2016.

Do you record of attendance till January End?

There is NO entry no.15.

Entry no;20 is for Rs.1025.85

Did you get salary slips of Dec,Jan?

Did anyone say HR decline to supply? 

sorry it was entry no. 5 for DEc and entry no. 20 for January, amount is erased due to personal.

I have n osalary slip and didnot ask to supply but if required, I can contact HR for the same.
