There is no format for appoinment order.Usually, an appointment letter is issued to the individual specifying the post to which appointed, the terms of appointment,salary,period of probation,the salary nature of appointment ie temporary or for a particular period etc., are mentioned.Normally the other benefits like ESI,EPF are not stated .
