What is the correct format of shuddhu patra

Hello,

i have purchased a plot in which my fathers name was written mistakenly. To correct, my lawyer have made a shuddhi patra on 500 rs stamp to present in UIT.

On 500 rs stamp my lawyer have written my name with my father incorrect name. And the person from i have purchased the plot.

and on other blank papers he has typed the request to change my fathers name from incorrect to correct. 

now i have confusion that on the first page (that is on 500 rs main stamp) the name written is still the same which needs to be corrected. And on other pages the request to correct the name is written

 

can UIT says that on the 500 rs main stamp you need to write the correct name in place of incorrect name. And reject the new shuddi patra.
