Consent terms filed, child custody? surname ?

Hie Need urgent Help

i have already got my consent terms filed which says child's costody will remain with me forever as i consider till age he is an adult,

i need to know know as per al this do i need to file for his custody papers of any more information to be added or any papers to be made for his & mine smooth life

Also need to know when my divorced is signed so i cage my parental name back i mean my name aswell my surname, i also want my sons surname to change as he is already 5 years & since birth he has not known nor met his father as per which i dont want hy ex husbands surname to be with my son ever

also as per this do i need any document or paper from the court to get conformation to change his surname

1 last ? & BIG confusion who is suppose to withdraw the 498a case form the high court as state court

The facts as stated are confusing that no definite and meaningful suggestion is possible unless the Judgment is seen.

Child name if u change to ur dads name it will appear as his child, n u ur father's wife. To change name u need to get consent of ex husband in writing through court of Law. Then can change name or kid should wait till it turn 18, n can apply for changing name through court. Notary change of name isn't valid. If u try to change name of kid without ex husband consent, he can always drag u n kid to court along with school authorities.
