Equitable Mortgage

I have been making an Deeed of Equitable Mortgage, but I did not  want to Notariesed (Notary) the Deed.

Thereafter I meet to Executive Magistrate (Tahasildar) I asked him about his Seal and Signature. The Executive Magistrate told me that He is ready if the Deed is in the form of affidavit.

Now How to make an Equitable Mortgage Deed, in Affidavit form for obtaining the Seal and Signature of the Executive Magistrate.

Sir,

Affidavit form as per me would be in the first person form ...... where the Mortgagor himself will state everyting on oath before the Executive Magistrate ........ I, so and so .......... resident of ........... declare on solemn affirmation that ..........
