Rules for election duty as polling officer for handicapped person

I am a teacher. Iam an orthopadeically (left leg short) handicapped 50% caused by Cerebral Palsy and also have Epilepsy.I received letter for 2018 Panchayet election duty as 2nd polling officer in West Bengal.I submit my prayer for exemption from duty with P.H certificate with all medical docs in District Polling Personnel office.They assure and receive it orally but not signed n release me in my duplicate application. Now what should I do? Go for 1st traning and at ten election? If I absent election duty for my health problem,what will be the punishment? Please reply. It's urgent for me to know. Thanks.
