Want to change surname & date of birth

Hi All,

I am Sanjay, as per title I want to change my surname & date of birth but I don't have procedural knowledge and documents detail require for proof. In my all documents my date of birth is 05-06-1983. I born in a village in Rajasthan on 11-03-1985 but don’t have any proof instead of Kundli.

Kindly suggest for evidence and procedure to change.....
