Hi,
I am a NRI applied for LIC housing loan. I was asked to give a power of attorney to my father in law in the format LIC provided. I am not sure what to be filled in the marked fields. And also not sure where my signature should be. Can some one help me here to understand? I have attached the picture for your reference.

You can clarify the details from the agent or the lic office itself or go through the form fully and understand the contents properly after which you will come to know what has been written in it and what is required to be done by you.
