Procudure to transfer property on death of member

I stay in Kalyan in a 2BHK flat with my mother & 2 elder brothers. Two years back my father died in a heart attack. The ownership of our flat was on my father's name however now we want to transfer the ownership of flat to our mother's name. So please guide us the procedure to transfer ownership of flat.
