Birth certificate

Hi all.. I havent registered my child's birth till now. I approached the municipal office, they gave me a form asking for the details that has to be furnished in the birth certificate. As my child was born.at home i have given my address in the place of birth column. Now my question is what documents will be needed by the court to grant birth certificate for my child. She is only 3years old, dont have any proof. I dont have ration card that contains her name. Please help
