I am giving you general guidelines it is better to approch a good advacate dealing in company matters.

Pass a resolution with majotity vote(75%) that the firm wants to change its constitutionin a company.

Authorise one or more partner to complete requsite formalities and apply or their digital signature.

All the partner of the present firm should be stakeholers in the new company having more then 51% share. 

Apply fo DIN no.

Search for company name 

Then apply for formation of company with asettlement deed prepared for taking over the business of firm.
