Change of Name to maiden name

Hi all, 

I want to change ma name back to my maiden name. All my legal documents ie, pancard, voters id , passport, ration card everywhere it is ma maiden name, 

I had used my husbands name only in ma work place, by submitting my marriage certificate. Also my salary account has ma husbands name as surname, However now i wana change my name back to my maiden both in office and n ma salary account. 

Does this require an affidavit.  Or it can be done only if I am divorced, Cant i retain my maiden name, if i am not divorced also

please advice,

you can change your name by sworing to an affidavit.  After this you will have to submit before your officer.

A very strange proposal. Sir names are meant to indicate a persons family status. For many legal and official purposes sir names are necessary. Does not ur propsoal mean that instead of ur husbands name u want to keep ur fathers name, as it was before ur marriage?  Not advisable.
