For conversion of religion , please approach to a priest of the religion you want to change and ask for the rites and ceremonies necessary to do so.  And I think you need to submit a declaration infront of a Magistrate and publish it in leading newspapers.

Though Jains are part of "hindu" religion, it may be proper for you to seek guidance from a Jain Monk as to the rituals to be undergone by you.
