Father's name mentioned wrongly in passport

hi all,

I had received my Passport in 2002 & got it renewed recently in 2012.

My father's name has been wrongly entered in my passport. The same wrong name appears in my Baptism & Birth Certificate also.

Now i need to correct his wrongly written name in my passport.

I would like to know how to go about this.

What documents are required to correct his name

his original name = estavaum

wrongly type name = stephen

His wrongly typed name reflects in the following documents

* His Birth Certificate

* His Passport

* My birth Certificate

* My Passport

Soeone kindly guide me how to start about correcting the name in my passport & Birth Certificate

TO MADE CORRECTION IN THE PASSPORT YOU HAVE TO APPROACH PASSPORT AUTHORITIES , BIRTH CERTIFICATE YOU HAVE TO APPROACH YOUR CORPORATION OR MUNICIPAL OFFICE.

meaning - Does my father have to correct his name first in municipality & then approach passport office?

Can he not just say that he wants to change his name & then once he get's his name changed i go to passport office and tell them to add my father's new name in my passport.
