Gratuity eligibility

I have worked in a private company which is having 5 working days for 4 yrs 7 months. My date of joining was 12 Nov 2010 and date of leaving was 15 jun 2015. Can you please let me know whether I am eligible for gratuity? I checked that being I'm continuous service for 4 yrs 190 days make one eligible. Can anyone please guide me on this? Thanks in advance.

4 Years 240 days is elgible for payment of bonus

Submit FormI under proper acknowledgment.

Dear Mr. Kumar, I received a reply that I am not eligible for gratuity.. Do you think that submitting form I will help me? And can you please explain what does proper acknowledgement means???
