How to file an i.a. before supreme court

I am working for envt. protection and afforestation in jabalpur, mp. I found here a forest land scam wherein the high profile people like son of Hon'ble Speaker of MP Vidhan Sabha is involved. I want to file an IA before Supreme Court in the case T.N. Godavarman Vs UoI. I have already registered with supreme court for e-filing but I am unable to find the format of IA. Anyone can help me??? Please provide me the format of IA which i have to present before supreme court. Thaning you.
