Id proof document

Hello All,

This question is regarding the ID proof document for my mother.

She has a Voter ID. However her surname is not included in it. Her name is Jyothi Prabhu and in voter id it is Jyothi. She does not have any other documents.

For opening a bank account, applying for pan card, senior citizen card etc, they are asking an id proof with full name.

Please advice on how to get an id proof with the full name. Can it be done through an affidavit ?
