Is FIR or MLC inevitable for claiming Medical Insurance

In a criminal attack on me, I suffered deep eye injury. Unfortunately, two of my family members were accomplices to this criminal.
I was hostage to these criminals for 3 months and as expected they never took me to a Doctor.
When ultimately I escaped from their capture and visited an Eye Surgeon, he proposed surgery costing 2 lakhs. The TPA asked 02 queries
1. Details of Incident
2. FIR/MLC copy or Declaration for Not Filing FIR

I gave a lucid account of things and declared that to protect my misguided family members who were accomplices , I prefer not to lodge FIR.

The TPA is stubborn about FIR/MLC.
Please guide.

Then ask the 2 lakh money from misguided family members who were accomplices instead of insurance.
