Name change in ssc certificate

I'm appeared for SSC examinations in 2007 fromAndhra Pradesh SSC Board and received the Certificates.In that Certificates my name "SYED SYFULLA is printed as "UMAR FAROOK'' date of birth is printed wrongly.I I want to correct it. So please tell me the procedure for name correction and date of birth and about the document required for the process.Apart from the SSC Certificate in all other documents my name ok .
