One sheet in my ia petition changed by court or my advocate

Dear Experts,

I filed a custody petition along with the visitation IA in salem FC in 2022. Main petition got numbered but IA was not. When I checked with my advocate she told there was no IA exist in the file as the office staff searched and said it was missing. Again I filed another IA mentioning the same point that "since my initial IA was missed and hence refiling the same again now". My IA was dismissed and I now move to HC for revision. As part of it , I took certified copy of all main, IA petitions along with the counter filed by the opposite party. Now in the IA page 13 was completely changed(only in page 13 my signature is not there) as 'since the initially submitted IA has been rejected, any order made ealrier would impact the future of petitioner and his son.'.

This was not filed by me.If it was rejected earlier,my advocate would have got me the copy and I would have already gone to the HC 2 years back. There is some malpractice happening in the court. What step should I take now?

You may move high court with a revision petition against the orders dismissing your IA for visitation rights and any other reliefs.

You don't have any evidence to prove malpractice hence no complaint would be maintainable.

But who has changed the reason "missing" to "rejected"? And there is no my signature as well. Is this not malpractice? then what else is malpractice?. So anyone can change any petition without petitioner's consent and run the case is it?
