Pan deactivated

Dear Experts,

 

I have been using PAN A for the last 11 years in a city, away from my hometown. Last week, suddenly my PAN got deactivated. I then got to understand that there is PAN B in my name with my hometown address. I was never aware of PAN B, didn't received this card and hence have never used.

What is the right procedure to get PAN B deactivated and then get PAN A activated?
